Traumatic Arteriovenous Fistula


Abstract
A 26 years old R.O.K.A. male was admitted to the department of Surgery of 17th Army Hospital with suspicion of traumatic arteriovenous fistula of the right neck and multiple shrapnel wound on the both lower extremities by shell fragments.
On admission, continuous machinery-like murmur with thrill and fist sized pulsating hematoma were noted on the ant. lat. aspect of right neck. Branham-Nicoladoni sign was also positive. Multiple shrapnel wound was healed completely with bed rest, local debridement, aseptic dressing and antibiotic use, but the arteriovenous fistula and peroneal nerve palsy of right lower extremity until repair of fistula was noted.
Right carotid angiography with 50% Hypque (30cc) was revealed the arteriovenous fistula between the right common carotid artery and internal jugular vein.
The repair of this arteriovenous fistula was performed by means of the division of fistulous tract with simple closures of common carotid artery and internal jugular vein.
The result of operation was good.
